Case managers engage with neighbors daily to complete both intake assessments and CES (Coordinated Entry System) assessments to help neighbors meet their needs. The intake process ensures a comprehensive assessment to identify appropriate short & long-term interventions.
Case managers also meet with neighbors to make immediate referrals for shelter or housing, medical care, mental health care, legal and vocational services. Additionally, the team works with neighbors to address their need to secure personal identification documents, clothing and transportation.
